Short description: Challenges: Ageing society, sustainability of health systems, and multimorbidity, including different disorders, e.g. dementia. In Estonia, there is an unmet need for home-based care services for the elderly – high fees, insufficient financial aid from local governments, too few specialists, etc.
Market potential: 148 care homes in Estonia, 8300 places for the residents. Number of dementia patients (22,942 in 2018) in care homes is unknown. Access and availability of healthcare are limited.
Timing: The challenge is imminent and issues ongoing, therefore, there is a clear need for new solutions in the field, as we speak. The challenge owner is ready to proceed to spearhead the solution.
Desired result: Significant improvement in health-realted outcomes; transparency of care service standard and simplier implementation, including adherence inside the care institution; reduced GP contacts; fewer avoidable hospital admissions, and less neuroleptic and/or sedative medication.
